چکیده

Abstract Background Mitral regurgitation (MR) is the second-most frequent valvular heart disease in Europe and frequently aggravates failure (HF) symptoms. transcatheter-edge-to-edge repair (TEER) can be considered eligible patients, for both primary (in inoperable cases) or secondary severe MR. However, intervention not advised comorbid patients whom it expected to prolong survival over 1 year. Purpose Evaluate characteristics associated with HF New York Heart Association (NYHA) class recovery, one-year all-cause mortality after mitral TEER Methods All procedures MR conducted a single-centre between 2014 2020 were retrospectively analyzed. The endpoint was defined as reduction of at least one NYHA first month intervention, recovery two classes. Survival status 12 months also consulted. Clinical, echocardiographic blood-analysis data explored endpoints defined, using Pearson's Chi-squared test, Wilcoxon rank sum test Fisher's exact appropriate. A p<0.05 statistically significant. Results From 103 procedures, 86 (83%) had full information about pre- post-intervention class, well months. There higher proportion among non-responders (47% versus 25%, p=0.034), but no differences Higher surgical risk (EuroSCORE II) tended have exhibit more though reaching statistical significance (p=0.068). Both advanced baseline lower N-terminal pro-brain natriuretic peptide (NTproBNP) levels linked symptomatic (2048 5676pg/ml, p<0.001). Also, persisting grade 3/4 non-NYHA responders. Regarding improvement classes, observed 13% these NTproBNP basal levels, exhibited estimated systolic pulmonary artery pressure inferior vena cava (IVC) diameter. Finally, 10 (11.6%) died months, significant associations found regarding pre-intervention survival. Conclusions This study suggests that three-fifths improve their TEER, only one-fourth Earlier – less hypertension, IVC diameters improvement. All-cause year still significant, exposing need better patient selection. findings represent exploratory deductions relatively low number, single-centre, patients. Predictors of in-hospital mortality after mitral valve surgery for post-myocardial infarction papillary muscle rupture

BACKGROUND Papillary muscle rupture (PMR) is a rare, but often life-threatening mechanical complication of myocardial infarction (MI). Immediate surgical intervention is considered the optimal and most rational treatment for acute PMR, but carries high risks. At this point it is not entirely clear which patients are at highest risk. In this study we sought to determine in-hospital mortality and...

Predicting 1-year mortality after hospitalization for community-acquired pneumonia

BACKGROUND Community-acquired pneumonia (CAP) is a major public health problem with high short- and long-term mortality. The main aim of this study was to develop and validate a specific prognostic index for one-year mortality in patients admitted for CAP. METHODS This was an observational, prospective study of adults aged ≥18 years admitted to Galdakao-Usansolo Hospital (Bizkaia, Spain) from...
